Identifying and addressing disparities in access to health care
Primary Care and Prevention research is devoted to helping people identify and address their risk of developing two of Australia’s most prevalent chronic diseases — cardiovascular disease and diabetes. The research conducts screening tests and develops nurse-led models of care to achieve ideal risk levels. The research aims to prevent these diseases particularly in disadvantaged and high-risk settings such as regional and remote communities and Indigenous communities. The research also develops electronic tools to assist health care professionals in managing and reducing individuals’ health risks.
